For Owner:

a. Land Title or TCT

For Lessee:

a. Contract of Lease (Notarized)

b. Building Owner’s Authorization Form

1. NBI Clearance (Current)
2. Brgy Clearance (Personal Record)
3. Sketch/Location Map of the Proposed Agency Site
4. Photograph of the Vicinity
5. Photograph of the Building
6. Photograph of the Interior

Corporate Applicant:

1. Bio-data of Corporate Officers and Directors
2. Photocopy of S.E.C Registration
3. Photocopy of Articles of Incorporation
4. Photocopy of By Laws Latest audited
